KidMin Information Form

This form is to get all the kids information we will need for our KidMin Programs! Please note the information on this form is for the use of Blessed Hope Community Church and is not available for other individuals and groups. We will not disclose your email address, cell number or any other details without your permission.

Any additional information you would like us to know about your child?

Would you want us to change diapers while they are in our care or would you prefer we page you if they need changed?

Would you be ok with us helping them or would you want to be paged down to help them?

We have snacks at different times during our KidMin Programs. Would you be ok with them participating in snack?

I, (parent/guardian below) give permission for my child attending BHCC for any function to being photographed.
I agree that BHCC shall have the right, but not the obligation to use my child’s photograph, for their website or social media account at any time and for any other purpose or materials the ministry deems necessary. The child’s name will not be used with the photos.

At times it becomes necessary to use private vehicles or the church-owned bus to transport children to and from BHCC children/youth activities. I, (parent/guardian below) hereby release BHCC, as well as its directors, employees, or volunteers from all liability or damages for any and all injuries arising from the negligence of any of the above while traveling to this activity via private vehicle or church bus.

In the event of an accident, injury, or illness, I (parent/guardian below) authorize any and all medical attention necessary to be administered to my child, under the direction of such medical professionals as the Church determines appropriate under the circumstance.

We care for the health and well-being of all children within our ministry, by asking that kids be kept home if they have an illness.
If a child has…
FEVER: Child should be fever-free for 24 hours without medication.
VOMITING/DIARRHEA: Child should be episode-free for 24 hours.
STREP THROAT: Child should have started antibiotics 24 hours before attending.
NASAL DRAINAGE: Child should not attend until clear.
SORE THROAT/COUGH: Child should not attend with a sore throat, croup, or persistent coughing associated with a respiratory infection.
PINK EYE: Attend only after 24 hours of antibiotics, no matting or drainage occurs, and the child is no longer rubbing the eye.
CHICKEN POX: Child may not attend until all spots are scabbed over and the child is no longer scratching or touching scabs.
RASH: Children with an unexplained or contagious rash should not attend.
LICE/ OTHER INFESTATION: Children should not attend until after treatment and all lice and visible nits have been removed.

If we don't have enough staff to cover a room, that room will need to be closed that day. We will hopefully have another option when this happens, but know that if your child is in a different room they might not be doing the same things they would have done in their usual room.
